Attention: The Sports Physical Form has Changed

The Pennsylvania Interscholastic Athletic Association (PIAA) requires ALL student-athletes to have a completed CIPPE form prior to participating in Inter-Scholastic sports in PIAA member schools.

The PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) plays a role in the student-athlete’s eligibility for the sport he or she chooses.  The PIAA also has strict guidelines concerning the CIPPE that must be followed in order to ensure that your son or daughter can participate in sport.

• The CIPPE shall be performed no earlier than June 1^st and shall be effective, regardless of when performed during a school year, until the next May 31^st.
  • For example, for the 2012-2013 school year, physicals may not be performed prior to June 1, 2012 and will effectively expire on May 31, 2013.
  • A physical performed in March for a spring sport will expire at the end of May.  If your son or daughter chooses to play a fall sport the following school year, another physical MUST be performed after June 1^st.
• THERE ARE NO EXCEPTIONS TO THESE RULES. 
  • This is a PIAA mandate, and as a PIAA member school, we must follow these guidelines.
• It is the responsibility of the parent/guardian to have the CIPPE completed for the student-athlete to participate in any Crestwood School District sport.
• Failure to follow these guidelines will delay the student-athlete’s participation in practices and games.
• Physical packets will be available in the high school and middle school offices, the athletic training room, and are also available online at www.csdcomets.org

The first 6 Sections need to be completed before your son or daughter participates. 

• Medical insurance is required to participate.

We recommend retaining a copy of Section 6 for your records.
